Our websites can provide both ‘Standard’ and ‘Custom’ secure online forms. ‘Standard’ forms describe the ready-made forms available within your website designed to deal with common patient-practice interactions, whereas ‘Custom’ forms are online forms you can create yourself to capture specific information you require from your visitors.
A range of pre-built standard secure forms are available for instant use. These include forms for repeat prescription requests, appointment booking or cancellation, patient registration and patient group enrolment, among others.
In addition to our standard secure forms, you are also able to build custom secure forms using the form builder tool.
If you do not want to build the forms yourself, that's fine. We will happily do it for you.
All data submitted by patients is encrypted. You will receive a notification that a form has been submitted to the website. This notification contains no patient details. To access submitted forms you simply sign into the website's secure form area.
Our survey builder lets you create, run and publish practice surveys whenever you wish. Alternatively, you can easily link the site to third party surveys.
